Moves Sports Player Terms, Conditions, Waiver & Release Form
Participation Requirement
You must be 18 years of age or older to participate in any Moves Sports LLC league, game, event, activity, or program.
By registering for or participating in any Moves Sports activity, I represent, warrant, acknowledge, and agree that:
- I am at least 18 years old.
- I understand that participation in recreational sports involves inherent risks, including but not limited to injury, illness, permanent disability, paralysis, or death.
- I voluntarily choose to participate and assume all known and unknown risks associated with participation.
- I agree to comply with all Moves Sports rules, policies, codes of conduct, facility rules, referee instructions, and league decisions.
- I am physically capable of participating, or I have consulted a physician and chosen to participate despite any medical conditions.
Assumption of Risk
I understand that participation in Moves Sports leagues, games, practices, events, and related activities may involve risks including, but not limited to:
- Physical contact with other participants;
- Collisions;
- Falls;
- Slips or trips;
- Weather conditions, including heat, rain, lightning, wind, or cold;
- Field, court, turf, facility, or playing-surface conditions;
- Equipment-related injuries;
- Negligent, reckless, or intentional acts of other participants, spectators, officials, or third parties;
- Lack of fitness, fatigue, dehydration, or pre-existing medical conditions;
- Transportation, parking-lot, pre-game, post-game, or social-event-related risks.
I knowingly and voluntarily assume all such risks, whether known or unknown, foreseeable or unforeseeable.
Release of Liability
In consideration of being permitted to register for, attend, or participate in any Moves Sports league, game, event, activity, or program, I, on behalf of myself and my heirs, executors, administrators, successors, and assigns, hereby release, waive, discharge, and hold harmless Moves Sports LLC and its officers, directors, members, employees, contractors, managers, volunteers, agents, subsidiaries, affiliates, successors, assigns, insurers, sponsors, advertising partners, promotional partners, field providers, facility owners, governmental entities, private property owners, referees, officials, vendors, and any party named as an additional insured under any applicable insurance policy, collectively referred to as the "Released Parties," from any and all claims, demands, causes of action, damages, losses, liabilities, costs, or expenses arising out of or related to:
- My participation in any Moves Sports league, game, practice, event, activity, or program;
- My attendance at any Moves Sports-related event or gathering;
- My use of any field, court, facility, parking area, or surrounding premises;
- Any injury, illness, death, property damage, or loss connected to my participation or attendance;
- Any acts or omissions of other participants, spectators, officials, volunteers, or third parties.
This release applies to claims arising in whole or in part from the ordinary negligence of the Released Parties to the fullest extent permitted by law.
Medical Treatment Authorization
I authorize Moves Sports, its representatives, officials, referees, volunteers, or facility personnel to seek emergency medical treatment for me if they believe such treatment is necessary or appropriate.
I understand and agree that I am solely responsible for any medical expenses, ambulance costs, emergency treatment costs, hospital costs, rehabilitation costs, or related expenses arising from my participation in any Moves Sports activity.
I further understand that Moves Sports does not provide medical insurance for participants and that I am responsible for maintaining my own health insurance coverage.
Concussion and Head Injury Policy
I understand that concussions and head injuries are serious medical conditions.
If I am suspected by a referee, league representative, teammate, opponent, volunteer, or other person of having sustained a concussion or head injury, Moves Sports may remove me from play immediately.
If removed for a suspected concussion or head injury, I may not return to play that same day and may be required to provide written medical clearance from a qualified healthcare professional before participating in any future Moves Sports activity.
I agree to comply with all concussion-related decisions made by Moves Sports, referees, officials, or facility personnel.
Indemnification
I agree to indemnify, defend, and hold harmless the Released Parties from and against any and all claims, demands, damages, liabilities, losses, costs, expenses, and attorney's fees arising out of or related to:
- My actions, conduct, or misconduct;
- My violation of Moves Sports rules, policies, or codes of conduct;
- My violation of facility rules or applicable laws;
- Any injury, damage, or loss caused by me to another person or property;
- Any claim brought by another person or entity arising from my participation in Moves Sports activities.
This indemnification includes reasonable attorney's fees, court costs, and expenses incurred in enforcing this agreement or defending against claims related to my participation.
Photo, Video, and Media Release
I fully authorize Moves Sports LLC, its subsidiaries, affiliates, sponsors, advertising partners, promotional partners, representatives, and assigns to photograph, record, film, livestream, publish, display, reproduce, distribute, edit, and otherwise use my name, image, likeness, voice, appearance, performance, and biographical information for advertising, marketing, promotional, commercial, social media, website, sponsor, highlight video, and trade purposes without further notice, approval, or compensation.
This authorization is perpetual, worldwide, irrevocable, royalty-free, and applies to all media now known or later developed.
I acknowledge and agree that any photos, videos, recordings, livestreams, or other media obtained or created by Moves Sports shall be the sole and exclusive property of Moves Sports and may be used, edited, published, broadcast, or otherwise distributed at Moves Sports' sole discretion, except where prohibited by law.
Code of Conduct Policy
I understand that Moves Sports has a Code of Conduct Policy in place, and I agree to comply with it.
As a member of the Moves Sports community, participants are expected to:
- Respect the Game Treat the game, the rules, and the spirit of competition with respect at all times.
- Respect the Officials Treat referees and officials with courtesy and respect, even when disagreeing with a call.
- Respect Opponents Do not taunt, mock, bait, threaten, or otherwise put down opponents. Foul, abusive, discriminatory, or racially motivated language has no place in Moves Sports leagues.
- Respect Yourself and Others Treat teammates, opponents, officials, spectators, volunteers, and staff with respect.
Moves Sports reserves the right to suspend, remove, or permanently ban any player, team, or spectator who violates this Code of Conduct.
Moves Sports Policies
By registering for this league, I understand and agree to all of the following terms, policies, and conditions.
Player Registration Policy
All players must register before the first day of matches.
Failure to have at least 12 players on the roster by the first day of matches may result in the forfeiture of upcoming games until the required players are added.
No unregistered player may participate in any Moves Sports match.
Jerseys Policy
All players must wear their own jerseys with their assigned numbers once numbers have been assigned. Players from the Away Team, listed first on the schedule, must wear white. Players from the Home Team, listed second on the schedule, must wear black.
Players are not permitted to play without their jersey. No exceptions.
If enough players fail to comply with the jersey policy, the team may forfeit the match.
Forfeit Policy
If a team needs to forfeit a game for any reason, the team must email ielbeck@movessports.com at least 72 hours before the scheduled game.
A $50 forfeit fine will be added to the team account if the forfeit is received within 72 hours of game time or is communicated incorrectly.
A no-show by a team will result in a $100 fine.
A second no-show may result in removal from the league and future ineligibility to register a team.
All fines must be paid by 12:00 p.m. the day before the team's next game to avoid forfeiture.
Sportsmanship Policy
Moves Sports endorses the principles of good sportsmanship and is committed to fostering a community of respect, tolerance, and safety for all participants.
Unsportsmanlike conduct will not be tolerated. Unsportsmanlike actions may include, but are not limited to:
- Fighting, pushing, or excessively violent fouls;
- Verbal or physical abuse directed toward an official, opponent, teammate, spectator, or staff member;
- Profanity or obscene gestures;
- Racially, ethnically, gender-based, or identity-motivated language;
- Taunting, trash talking, baiting, or physically intimidating another person;
- Cheating;
- Throwing, damaging, or abusing equipment;
- Refusing to follow referee, official, facility, or league instructions.
A player may be ejected from a game, and a team may be removed from the field of play, for engaging in unsportsmanlike conduct.
An official may call a forfeit for unsportsmanlike conduct at any time, including after the game clock has expired.
Moves Sports has the authority, in its sole discretion, to suspend or expel a player, team, or spectator from the league and/or future games without refund.
These guidelines apply to players, teams, and spectators.
Weather & Game Reschedule Policy
Severe Weather Forecasted: If severe weather is in the forecast, Moves Sports will monitor the situation throughout the day and evening. If games are cancelled, impacted players will receive an email notification as soon as the decision is made.
Weather at the Field: Sometimes only certain fields may be impacted, or storms may occur quickly near game time. In those situations, officials at the field have discretion regarding whether play should continue, be delayed, or be suspended.
If any individual involved with the league, including players, referees, spectators, or volunteers, sees lightning or hears thunder, that person must immediately bring it to the referee's attention.
When a referee sees lightning, hears thunder, or is alerted to lightning or thunder, the referee will immediately suspend play for 30 minutes. Everyone must clear the field and retreat to a safe area, such as the parking lot or another designated location. Anyone who remains on or near the field after play has been suspended does so at their own risk.
No individual may ask or pressure a referee to ignore lightning or thunder or to allow play to continue. The referee will wait 30 minutes from the last flash of lightning or sound of thunder. Each time lightning appears or thunder is heard, the 30-minute clock will restart. Actual game time may be shortened to allow the game to be completed on the impacted night.
Reschedules: If a game is not played due to weather, Moves Sports will attempt to reschedule it later in the season. Moves Sports reserves the right to run a shortened season without refund.
Jewelry, Electronics & Supportive Device Policy
For any Moves Sports match, players must remove all exposed facial jewelry, watches, cameras, GoPros, cell phones, activity trackers, wrist jewelry, and similar items.
Medical ID bracelets may be worn at any time.
A guard, cast, or brace made of a hard or unyielding substance, including plastic, plaster, or metal, must not be worn on the elbow, hand, wrist, or forearm, even if covered by padding.
Knee and ankle braces are permitted and do not require padding unless otherwise determined by the referee.
All jewelry and supportive equipment decisions are at the referee's sole discretion.
Facility / Field Use Policy
I understand that Moves Sports has a Facility and Field Use Policy in place, and I agree to comply with it. Participants, teams, and spectators must:
- Stay off the field until the prior user group is finished;
- Follow all facility, school, park, field, and property rules;
- Refrain from bringing alcohol or drugs on or near the field;
- Refrain from smoking on or around turf fields;
- Keep pets off turf fields and school grounds unless expressly permitted by the facility;
- Clean up all trash, equipment, and personal belongings;
- Respect facility staff, referees, officials, and other users.
Failure to comply with these rules may result in forfeiture, suspension, removal from the facility, or removal from the league.
League Format and Competition Changes Policy
Moves Sports reserves the right, in its sole discretion, to modify any aspect of the league, competition, event, or related activities at any time if reasonably deemed necessary for operational, financial, safety, legal, facility, weather, sponsorship, insurance, participation, or other business reasons. Such modifications shall not constitute a breach of this agreement and shall not entitle any participant to a refund, credit, damages, or other compensation, except as otherwise required by applicable law.
Refund Policy
All registration fees are non-refundable. Registering for a team is a commitment to participate in the full season.
In some cases, Moves Sports may need to delay the start of a league. This may be due to teams needing more time to complete rosters, facility scheduling conflicts, weather, field availability, operational issues, or other circumstances. Moves Sports reserves the right to delay league starts and will attempt to incorporate all planned games into the revised schedule to the best of its ability. Delayed season starts are not grounds for a refund.
In the case of cancelled games due to weather, facility issues, scheduling limitations, or any other circumstances causing a reduction in the number of games played, there will be no refund or credit for games not played.
Moves Sports reserves the right to host games on days other than the normal day of regular season play and at alternate times or locations from those originally designated if necessary.
Refunds will only be processed at the discretion of Moves Sports and will generally only occur if an individual league or activity is cancelled due to an insufficient number of teams registered.
Changes in location, start date, end date, season length, game time, or duration are not grounds for requesting a refund.
All refund requests must be submitted in writing to ielbeck@movessports.com and must be received before the originally posted registration close date for that league.
All social event tickets are non-refundable.
Final Acknowledgment
I acknowledge that I have carefully read this Player Terms, Conditions, Waiver & Release Form.
I understand that by registering, e-signing, or participating, I am giving up certain legal rights, including the right to bring certain claims against Moves Sports and the Released Parties.
I confirm that I am entering into this agreement voluntarily and that no oral representations, statements, or inducements apart from this written agreement have been made.
I agree that this agreement shall be binding upon me and upon my heirs, executors, administrators, successors, and assigns.
